Information to 3rd Quarter AY2021-2022 Graduating Students

1. DIPLOMAS and TRANSCRIPT OF RECORDS

   The start of application for diploma and transcript of records is on July 4, 2022. Processing of documents are on a “first come, first served” basis.

   Diplomas are issued only once. In case of loss, a letter of request is required to obtain a duplicate copy.

2. FOR THE BOARD EXAMS

   The Professional Regulation Commission (PRC) requires the transcript of records (with recent photo) of graduates who are applying to take board examinations.

   Student accounts in all departments must be cleared before the issuance of any certification.

3. CARDINAL AND GOLD

   Subscription to the Cardinal & Gold (Yearbook) is compulsory.

4. APPLICATION FOR TOR AND DIPLOMA
